Botox® Cosmetic and Dysport® are both medicines that reduce unwanted lines and wrinkles in specific areas of the face by quieting the underlying muscle activity. In areas such as the forehead, glabella (area between the eyebrows), crows feet and areas around the mouth, the skin and underlying muscle connect so that when the muscle tightens, the skin overlying that specific muscle folds. As skin loses elasticity, frequent folding of the skin will eventually leave a crease in the skin even when the muscle is at rest. To improve the lines in the above-mentioned areas, it is critical that the muscle activity be controlled. Botox® Cosmetic and Dysport® are chemically identical neuromodulator proteins that are manufactured to block the nerve endings release of the neurotransmitter acetylcholine so that muscle contracture cannot occur. The beneficial effects of these medicines first appear about 2-3 days after injection and last on average between 3 and 6 months. To maintain the relaxation of the facial muscles that have been treated, retreatment is required. If the effects of the medication are allowed to go away completely, the muscle will simply return to it normal activity level unless the muscle has been kept “asleep’ continuously for about 2 years or more. In that circumstance, the muscle may have atrophied to the point that some residual weakness may persist even if these medications are not continued. Though this effect may be deemed desirable by some since continued treatment with either Botox® Cosmetic and Dysport® may not be necessary, this cannot be reliably predicted.

Additional aesthetic uses for Botox® Cosmetic and Dysport® include balancing facial muscle activity in patients with facial paralysis, “lifting” of the eyebrows and corners of the mouth, “opening” the eyes in patients whose eyes become excessively squinted with smiling, controlling “bunny lines” on the nose, reducing the lateral fullness of a face by treating the masseter muscles, reducing chin dimpling and dropping the upper lip to reduce excess maxillary gum show.

Dr. Yellin is a nationally recognized expert in neuromodulators, medicines that temporarily reduce muscular activity. As a member of the National Education Foundation, he teaches other physicians throughout the country the proper techniques for using Botox® to enhance facial appearance.

When creating a youthful facial appearance, it is no longer acceptable to address only skin sagging. The treatment of unwanted lines and wrinkles is essential. Also, the magical third component of facial rejuvenation, the replenishment of lost  and redistributed facial volume. Several different FDA-approved injectable materials are available for line and wrinkle therapy and facial-volume enhancement. Products such as Juvederm Ultra®, Juvederm Ultra Plus®, Restylane®, Perlane®, Belotero Balance® and Radiesse® are all dermal fillers intended to create volumetric enhancement of the skin and underlying soft tissues, that will last on average between nine months and two years, depending on what product is injected and its location on the face.

Sculptra® is a product that stimulates a patients own collagen production to gradually and naturally restore volume loss over a period of about 6 months. The average patient will require 2- 4 treatments spaced 6 weeks between each treatment, Since it is the patients own collagen that will ultimately replenish facial volume, the effects may last for several years. However, unlike the injectable filler materials mentioned above, millimeter specificity cannot be controlled since the patients response to Sculptra® may vary. For this reason, Dr. Yellin prefers to use Sculptra® in the lateral face such as the temple and cheek areas rather than the midface where millimeter volumetric control is critical.

LaViv is a personalized biological product that is available to treat unwanted facial lines and wrinkles using your own collagen producing cells known as fibroblasts. The procedure requires that Dr. Yellin take several small skin biopsies from behind your ear and then send these off to a lab where the fibroblast cells are nurtured and multiplied. Several months later, these cells are sent back to Marietta facial Plastic Surgery & Aesthetics Center on a prearranged day. Dr. Yellin will then inject your own cells into the unwanted lines and wrinkles and overtime, these cells will produce collagen and reduce the appearance of the treated lines and wrinkles.

 Ultherapy® 

At Marietta Facial Plastic Surgery & Aesthetics Center, we offer Ultherapy® for non-surgical tightening and lifting of the face and neck. Ultherapy® is the first FDA approved non-invasive treatment to tighten and lift the jawline and neck and the brow.

The ideal patient for Ultherapy® treatment is an individual who has early to moderate facial and neck skin sagging or brow droopiness or has previously undergone facial aesthetic surgery and wishes to enhance his/her facial appearance. This is a procedure that is performed with no anesthesia in about 2 hours, provides gradual but meaningful improvement that peaks at about 6 months after treatment, with results that last for approximately 3-4-years. There is no bruising or swelling and no post treatment downtime and is available at a fraction of the cost of surgical treatments.

Ultherapy® uses focused ultrasound energy that can be targeted to multiple layers of the face and neck including the skin, subcutaneous fat, fascia and muscle layers to create intense heat resulting in controlled tissue injury which the body responds to with collagen production and tissue contracture. The Ultherapy® machine also has a display which allows the technician to visualize the exact layer within the face and neck that the energy is being delivered to, which provides excellent control and safety during the procedure.

All of this results in meaningful tightening and lifting of the treated tissues, which can last for years. Two weeks prior to treatment, you should avoid medications and oral supplements that are known to facilitate bruising such as Aspirin, NSAIDs such as Advil, Nuprin, Aleve and Motrin, Vitamin E, Fish Oil, Gingko, Ginseng and Ginger tablets. On the day of your procedure, after arriving at the office, you will be asked to sign the procedure consent and have your photos taken. Following this, you will take a Valium and a Hydrocodone/Acetaminophen tablet that you will have purchased at the drugstore prior to your treatment. You will then rest in our massage chair until you are relaxed. Once relaxed, you will be transferred to the treatment room and the Ultherapy® procedure performed. It takes about 2 hours to treat the entire face and upper neck. Less time is needed if only a facial segment is being treated. Once the treatment is complete,  you must have a driver take you home due to the pre-medications used. No down time or recovery is expected. All of the important healing and tightening occurs deep to the outer layers of the skin. Routine office follow-up is at 3 and 6 months.
