1

Make Contact

The first step is to call our office at 770-425-7575 or complete the “Schedule A Consultation” form below. If you are requesting an on-line, virtual consultation (we use Doxy.me which is HIPAA compliant), please contact our aesthetic coordinator so they can coordinate a time for you and Dr. Yellin to communicate.

2

Send Your Photos

All long-distance patients should provide clear, recent full-face (frontal, left and right ¾ and left and right profile views) and close-up photos that clearly show your area(s) of concern and can be sent by regular mail or e-mailed to Dr. Yellin at dhatten@mariettafacialplastics.com.

3

Obtain an Assessment

Dr. Yellin will meet with you virtually using the Doxy.me virtual, HIPAA compliant platform to assess your aesthetic concerns and review your medical history. He will then determine the best course of treatment and may also raise issues that you had not considered but are important to address in order to create a naturally beautiful outcome. Dr. Yellin will then summarize his treatment plan with you.

You will then be contacted by our aesthetic coordinator who will provide you via email, a written quote for your recommended procedure(s). Our aesthetic coordinator will also review with you pre-operative planning, recovery, local accommodations, and any questions related to your procedure(s).

4

Schedule Your Procedure

Once you have decided to proceed, a 20% deposit is required to reserve your surgery date. This is non-refundable after 7 days from scheduling. The remaining balance is due two weeks prior to your surgery date.

You will then be scheduled for a virtual pre-operative visit with one of Dr. Yellin’s assistants, who will review all of the details of your upcoming procedure(s), including the risks, benefits and limitations of each procedure, pre and post-operative care and what to expect during the healing process. This will also be an opportunity to have any of your procedure-related questions answered.

5

Medical Clearance

All patients having general anesthesia and who are over the age of 60 or at the discretion of Dr. Yellin are required to have an EKG within 6 months and blood work within 30 days of their procedure and a letter of medical clearance from their PCP.

6

Read, Initial, Sign

Shortly after your surgery is scheduled, you will receive a packet in the mail containing pre-operative instructions and procedure consents. These documents must be read carefully and all instructions followed closely. These documents will be reviewed with you at your virtual pre-operative visit.

Once you feel that you are fully informed and that all of your procedure related questions have been answered satisfactorily, you will need to sign and return the completed consents to us either by mail or, you may scan them and email them to us at elester@MariettaFacialPlastics.com. All consents must be signed in the presence of a notary public, and notarized (if domestic), or authenticated by an official witness (if international). If you have questions at this time please call or email our office.

Fly in for Surgery

The nearest airport to our office is Hartsfield-Jackson Atlanta International Airport. We are located approximately 32 miles north of the airport. You MUST arrive one to two days prior to your surgery date. At a designated time during this period you will meet with the doctor and have an in-depth consultation reviewing your upcoming procedure(s). At this time you will sign your surgery consents (if necessary) – in the presence of our patient care coordinator.

8

Recovery

We advise all patients that fly-in for surgery to spend at least the first night after surgery with our overnight care provider at a hotel near the surgical center, thus ensuring you are cared for properly. In the days that follow, we will see you post-operatively and depending upon the procedure(s) performed, you may return home in 7-10 days from surgery. Our staff will assist you in making a local hotel reservation for your stay. In the event that you require any medical attention after returning home, we will refer you to a local physician for evaluation and care. Your health and safety remain our number one priority.

Our entire staff looks forward to meeting you and making your time with us stress-free.
