Injecta-Lift ™ | Using Facial Fillers

When creating a youthful facial appearance, it is no longer acceptable to address only skin sagging and unwanted lines and wrinkles. The critical third component of facial rejuvenation is replenishment of lost and redistributed facial volume. Depleted facial volume is responsible for hollowness, as well as the root cause of many of the sags and bags associated with aging.

Dr. Yellin has developed the Injecta-Lift™ technique, which combines various hyaluronic acid (HA) fillers, placed deeply, on the bone when possible, to 3-dimensionally and comprehensively reshape and rejuvenate the face to create a long-lasting change that is totally natural appearing and long-lasting. The Injecta-Lift™ technique can be used on its own or as a complement to other surgical interventions to optimize results. Used in combination with other techniques, it allows the shape of the face to complement the texture and tightness of the the rejuvenated face.

Many different FDA-approved injectable materials are available for facial-volume enhancement. Most of the fillers that Dr. Yellin uses are made of hyaluronic acid (HA). Though they have the same general chemical composition, there are qualitative differences that make some fillers more firm and designed to lift heavier soft tissues, while others are more flexible and best used in areas of frequent movement such as around the mouth and still others have a  softer feel making them ideally suited for the thin, looser soft tissues under the eye. Products such as Juvederm Ultra®, Juvederm Ultra Plus®, Volux®, Voluma®, Vollure®, Vobella®, Restylane®, Restylane Lyft®, Restylane Silk®, and Kysse® are all examples of HA fillers. The longevity of any filler correction is variable and depends on the patients biology since these fillers are degraded both enzymatically and mechanically. However, by placing the fillers deeply and providing full volumetric correction, Dr. Yellin’s patients can often retain there improved appearance for years following their Injecta-Lift™ procedure.

Radiesse® (calcium hydroxylapatite) is also a dermal filler and can be used as such. However, Dr. Yellin currently uses Radiesse® as a hyper-dilute solution which stimulates collagen production to thicken the neck skin, improving its appearance. Radiesse® is often combined with Profound RF (see below) to improve the texture and contour of the neck skin non-surgically.

Botox® Cosmetic & Dysport®

Dr. Yellin is a nationally recognized expert and national trainer in the proper and creative use of neuromodulators such as Botox® Cosmetic and Dysport®. These medications reduce unwanted lines and wrinkles in specific areas of the face by quieting the underlying muscle activity. In areas such as the forehead, glabella (area between the eyebrows), crows feet and around the mouth, the skin and underlying muscle connect so that when the muscle tightens, the overlying skin folds. As skin loses elasticity, frequent folding of the skin will eventually leave a crease or line, even when the muscle is at rest. To minimize the lines in these areas, it is critical that the muscle activity be controlled.

Other creative aesthetic uses of Botox® Cosmetic and Dysport® include lifting of the lateral eyebrows and corners of the mouth, keeping the eyes more open in patients whose eyes become excessively squinted with smiling, controlling bunny lines on the nose, reducing the lateral fullness of a face by treating the masseter (chewing) muscles, reducing chin dimpling and excess chin projection, dropping the upper lip to reduce excess maxillary gum show (a gummy smile), balancing facial muscle activity in patients with facial paralysis, reducing facial sweating when eating (a complication of surgical removal of the parotid gland) and, reducing the appearance of platysmal neck bands.

So, how do Botox® Cosmetic and Dysport® work? They are chemically identical neuromodulator proteins that are manufactured to block the nerve endings’ release of the neurotransmitter acetylcholine so that muscle contracture cannot occur. The beneficial effects of these medicines first appear about two to three days after injection, are fully in effect about 1 week after injection and last on average between three and four months.

To maintain the relaxation of the facial muscles that have been treated, retreatment is required. If the effects of the medication are allowed to go away completely, the muscle will simply return to normal activity unless the muscle has been kept asleep continuously for about two years or more. In that circumstance, the muscle may have atrophied to the point that some residual weakness may persist even if these medications are not continued. Though this effect may be deemed desirable by some since continued treatment with either Botox® Cosmetic and Dysport® may not be necessary, this cannot be reliably predicted.

Sculptra®

Sculptra® is a product that stimulates a patient’s own collagen production to gradually and naturally restore volume loss over a period of about six months. The average patient will require two to four treatments, spaced six weeks between each treatment.

Since it is the patient’s own collagen that will ultimately replenish facial volume, the effects may last for several years. However, unlike the injectable filler materials mentioned above, millimeter specificity cannot be controlled since a patient’s response to Sculptra® may vary.

For this reason, if Dr. Yellin recommends Sculptra® it will typically be in the lateral face, such as the temple and lateral cheek, rather than the midface where millimeter volumetric control is critical. During your personalized consultation, Dr. Yellin will apply his knowledge and expertise to create a treatment plan that is specific to your needs.

Kybella™

KYBELLA™ (deoxycholic acid) is the first and only FDA-approved non-surgical injectable treatment used to reduce moderate to severe excess fat beneath the chin (submental fat) also called a “double chin”, resulting in a visibly improved chin and neck profile.

Many patients experience a meaningful contour improvement of their chin profile after two to four KYBELLA treatment sessions spaced at least one month apart. However, up to six treatments may be necessary to achieve the desired result. Non-Invasive Face & Neck Treatment

Ultherapy®

At Marietta Facial Plastic Surgery, Laser & Aesthetics Center, we offer Ultherapy® for non-surgical tightening and lifting of the face and neck. Ultherapy® is the first FDA approved non-invasive treatment to tighten and lift the jawline, neck, brow and décolleté.

The ideal patient for Ultherapy® treatment is an individual who has early to moderate facial and neck skin sagging, brow droopiness or laxity of the décolleté or has previously undergone facial aesthetic surgery and wishes to enhance his/her facial appearance. This is a procedure that is performed with no anesthesia in about 2 hours, provides gradual but meaningful improvement that peaks at about 6 months after treatment, with results that last for approximately 3-4 years. There is no bruising or swelling and no post treatment downtime. Lastly, though Ultherapy® is a very useful and effective technology, it is intended to treat mild to moderate skin laxity and is not expected to provide equivalent results to surgical procedures that lift, reposition and remove excess skin if that is required. 

Ultherapy® uses focused ultrasound energy that can be targeted to multiple layers of the treated area including the skin, subcutaneous fat, fascia and muscle layers to create intense heat resulting in controlled tissue injury which the body responds to with collagen production and tissue contracture. The Ultherapy® machine also has a display which allows the technician to visualize the exact layer within the treated area that the energy is being delivered to, which provides excellent control and safety during the procedure. All of this results in meaningful tightening and lifting of the treated tissues, which can last for years.

Before Your Ultherapy Procedure

Two weeks prior to treatment, you should avoid medications and oral supplements that are known to facilitate bruising, such as aspirin; NSAIDs like Advil, Nuprin, Aleve and Motrin; Vitamin E; Fish Oil; Gingko; Ginseng and Ginger tablets.

On The Day of Your Ultherapy Procedure

After arriving at the office for your procedure, you will be asked to sign the procedure consent and have your photos taken. Following this, you will take a Valium and a pain management tablet that you will have purchased at the drugstore prior to your treatment. You will then rest in quietly until you are relaxed. Once relaxed, the Ultherapy® procedure will be performed by one of our trained and certified aestheticians.

It takes about two hours to treat the entire face and upper neck. Less time is needed if only a facial segment is being treated. Once the treatment is complete, you must have a driver take you home, due to the pre-medications used. No downtime or recovery is expected. All of the important healing and tightening occurs gradually, deep to the outer layers of the skin. Routine office follow-up is at three and six months.

Minimally-Invasive Face & Neck Treatment

Profound RF

Profound RF is the latest development in wrinkle reduction and skin tightening technology. It utilizes radio-frequency (RF) energy to heat the dermal layer of the skin to a predetermined temperature for a pre-determined amount of time, set by Dr. Yellin based on your clinical circumstances. Over time, your body begins producing new elastin that in turn tightens and smooths skin in the treated area(s). By delivering radio-frequency energy through the epidermis and into the dermis using 5 pairs of micro-needles, Profound RF can safely heat the treated area(s) to an optimal temperature for elastin production and skin tightening while protecting the outer skin layers. This prevents blistering or peeling and shortens the healing time to about 1 week. Though downtime is minimal, final results will reveal themselves over a 6-12 month period of time. If necessary, fat reduction can also be achieved at the same time by using a needle system consisting of 7 pairs of needles directed through the skin and into the subcutaneous (beneath the skin) fat. Dr. Yellin currently offers Profound RF treatments for the face, neck, arms, legs and, torso. Facial treatments are done exclusively by Dr. Yellin while his physician assistant, Kelly Houmand, PA-C, performs our body treatments.

During your procedure, you will be relaxed using oral Xanax and locally anesthetized using injectable lidocaine. Once numb, many patients fall asleep during the treatment. There is some moderate swelling and skin redness for a few days after the procedure. Most patients experience little to no discomfort after the procedure. The length of the treatment session depends on the size of the treatment area but typically does not exceed 90 minutes. Patients can return to normal activities after their appointment. Results often last 3-5 years, though individual results may vary.

Lastly, though Profound RF is a very useful and effective technology, it is intended to treat mild to moderate skin laxity and is not expected to provide equivalent results to surgical procedures that lift, reposition and remove excess skin if that is required.
