Most people don’t wake up one day and decide they want eyelid surgery. They wake up and notice they look tired, despite feeling fine. Or, they catch themselves on a Zoom call and wonder when their eyes started telling a different story than the rest of their face.
The eyes are often the first place aging shows up. For many of us, we begin to recognize these changes in our 40’s. The eyelid skin thins, fat protrudes, puffiness lingers, and the lids begin to droop. The excess, inelastic, subtly wrinkled skin reduces the visible, clean upper lid platform. It also interferes with the pleasing aesthetic frame that the eyelids create around our eyes which makes our eye appear smaller and less engaging. Before you realize it, the message your face may send is weary, stressed, and older, regardless of how well you sleep or how you actually feel.
That gap is what drives the growing interest in blepharoplasty. For most patients, the motivation is simple: they want their eyes to look like they used to… open, alert, and rested. In just the past several months, five of my friends have seen me for just these issues and are now enjoying their refreshed appearances, with several of them sharing with me how emotionally impactful it has been, which was the motivation for this article.
Blepharoplasty, commonly known as eyelid surgery, is a targeted procedure designed to address sagging upper lids and under-eye bags. When skin is removed, incisions are placed discretely within the natural crease of the upper eyelid or just below the lash line on the lower lid, allowing scars to fade into near invisibility. When I remove lower lid fat, my approach is through the inside of the lower eyelid which avoids visible scars and virtually eliminates undesirable eyelid rounding which is a well-known risk of traditional lower lid surgery. The results are tailored to each patient’s anatomy to create a look that mimics their younger self, without changing how they look. The change is beautiful, natural, and often last for 20 or more years. What makes blepharoplasty compelling isn’t that it transforms you. It’s that it restores you.
I often combine eyelid surgery with additional procedures such as under eye and midface volume replacement to address the fat and bone loss that create a sunken, hollow look that often accompanies eyelid aging. Deeply placed hyaluronic acid fillers are my go-to technique for volume replacement, as they allow me millimeter control to artistically sculpt the area, with results that often last for years. Carbon dioxide laser resurfacing is another treatment I employ when appropriate, to rejuvenate the eyelid skin without the need for cutting or scarring. These results routinely last for 10 to 15 years.
Another reason for blepharoplasty’s popularity is practicality. All of the procedures discussed, except for lower lid fat removal, are routinely performed in-office under local anesthesia along with some oral Xanax to relax you. There’s no need for general anesthesia except for lower lid fat removal or when combined with a facelift, and no dramatic disruption to daily life. Sutures are routinely removed ten days after surgery. Most patients return to normal routines within several days, with swelling and bruising improving over one to two weeks. For professionals and busy parents alike, that matters.
Perhaps most important, when done well, blepharoplasty doesn’t advertise itself. You won’t look “operated on.” You’ll look like you slept well. Like you’ve been on vacation. No one can quite say what’s changed. That subtlety is powerful. When the eyes look refreshed, the entire face changes. Makeup sits better. Confidence rises quietly; not from vanity, but from fact that your appearance finally matches how you feel.
Of course, not everyone is a candidate. The best outcomes occur in healthy individuals with realistic expectations who are bothered by excess upper lid skin, under-eye bags, or both. A thoughtful consultation is essential to determine whether the concern is truly eyelid-related or influenced by other factors. Precision matters. Millimeters matter. For the right person, blepharoplasty can be one of the highest-impact, lowest-drama procedures in aesthetic medicine. It simply restores clarity to the most expressive part of your face, and sometimes, that’s all that’s needed.
