The Direct Neck Lift: A Targeted Solution for Neck Rejuvenation

In my December 2025 newsletter, I discussed the advantages of the sophisticated and aesthetically powerful deep-plane lower face and neck lift, which is the procedure I perform for comprehensive face and neck rejuvenation. For this month’s discussion, I will focus on a procedure that is less well-known, the direct neck lift.

While some patients complain of aging throughout the lower face and neck, others are primarily bothered by age-related changes in the neck and jawline—such as loose skin under the chin, excess fullness due to unwanted fat, visible vertical platysmal bands, jowls, a softened jawline, and a sagging neck angle. I often hear something along the lines of “can’t you just cut it out” while a patient grabs their “turkey” neck. Well I am here to tell you that yes, I can. This is the essence of the direct neck lift. It is a procedure that I learned during my facial plastic surgery fellowship thirty years ago from my mentor and professor Dr. Richard Farrior. During my time in practice, I have refined the original technique, and in 2014 I wrote a journal article that was well-received by my colleagues highlighting my modifications of the direct neck lift. To this day, I often get referrals from other facial plastic surgeons who are not well-trained in this procedure.

The direct neck lift removes the sagging, inelastic central neck skin and underlying fat directly and tightens the neck muscles along their entire length, from under the chin to the collar bone. This dramatically reduces the visibility of anterior neck bands, while creating muscular support that results in a sculpted, defined, youthful-appearing neckline. My excisional design also reduces the appearance of the jowls while tightening the jawline. For patients with isolated neck and jawline aging, a direct neck lift can deliver powerful results through a straightforward, targeted approach. Another very important advantage of this procedure is that the results last for many years as the most inelastic central neck skin is removed.

Strengths of the Direct Neck Lift

1. Focused Improvement for the Neck

Ideal for patients whose primary areas of concern are their jowls, softening of the jawline, the region beneath the chin, and a sagging neck. This approach delivers direct and noticeable enhancement where it’s needed most.

2. Addresses Platysmal Bands Effectively

For individuals with pronounced neck banding, the direct neck lift allows me to tighten or modify the medial platysmal muscle along its entirety, producing a smooth and supported neck contour.

3. A Shorter, More Streamlined Procedure

Because it focuses on a single anatomical area, some patients appreciate the more limited scope of surgery and recovery compared to more comprehensive approaches. It is routinely performed in the office using local anesthesia and oral Xanax to relax, and is ideally suited for those patients who medically are not good candidates for or simply don’t want general anesthesia. There are no bandages, and usually minimal discomfort while healing. Lastly, the procedure time and cost are about half of a deep-plane facelift.

4. Works well as part of a comprehensive face and neck rejuvenation plan

To minimize healing and downtime, a direct neck lift can surgically rejuvenate the neck and jawline and simultaneously, CO2 laser skin resurfacing can be done to tighten the facial skin while also dramatically improving lines, wrinkles, and blotchy brown discoloration. It can also be combined with the Injecta-Lift procedure to non-surgically replenish lost facial volume to rejuvenate the face.

Two Distinct Procedures, Each with Meaningful Benefits

Both the deep-plane facelift and the direct neck lift offer compelling advantages for the right patient. Whether someone seeks comprehensive rejuvenation of the lower face and neck or focused refinement of the neck alone, today’s surgical techniques allow for natural, long-lasting, and confidence-boosting results tailored to individual goals.

Rather than focusing on which procedure is “better,” it’s more helpful to understand what each procedure is designed to accomplish. Both procedures can restore confidence and create meaningful, natural-looking rejuvenation when chosen thoughtfully and performed skillfully. Since there is no such thing as the one perfect procedure for all patients, these two techniques make different trade-offs, each having their strengths and weaknesses. The important take-away is to figure out what procedure is best-suited to address your aesthetic concerns while respecting your personal constraints.
